Effect of short term sibutramine on gonadotropin spontaneous pulsatile release in normogonadotropinemic amenorrheic subjects

[Abstract]Objective To evaluate the effect of sibutramine on luteinizing hormone(LH) secretion. Methods Setting :amenorrheic patients in a clinical research environment. Patients :10 healthy normogonadotropic amenorrheic volunteers were enrolled after informed consent. Interventions :All patients underwent hormonal evaluations, an oral glucose tolerance test (OGTT), a naloxone test, a pulsatility study and a gonadotropin-releasing hormone(GnRH) stimulation test before and after 7 d of sibutramine administration (10 mg/d). Main outcome measures: plasma LH,follicle stimulating hormone, prolactin(PRL), estradiol, 17-hydroxy-progesterone, androstenedione, testosterone, glucose,insulin, C-peptide concentrations, BMI, HOMA index and glucose-to-insulin ratio. Pulsatility study and instantaneous secretory rates(ISR) were computed using the DETECT algorithm. Results After sibutramine administration, plasma LH increased, PRL decreased, insulin sensitivity, expressed as glucose-to-insulin ratio and HOMA index resulted significantly improved. While LH pulse frequency and LH response to GnRH bolus did not change under sibutramine adminstration, LH pulse amplitude increased as well as LH response to naloxone infusion. Conclusion Sibutramine admnistration allows to suggest that the negative modulation of the opioidergic pathway on GnRH secretion is under the influence of the serotonergic system.
